Non-Emergency Medical Transportation Services
AGE Medical Transportation LLC provides safe, reliable, and timely Non-Emergency Medical Transportation (NEMT) for clients who need assistance traveling to and from medical appointments. Our trained and background-checked drivers ensure a comfortable and dignified transportation experience every step of the way.
Step 1: Levels of Service
Ambulatory
For individuals who are able to walk on their own or with minimal assistance. Perfect for routine medical visits, follow-up appointments, and outpatient procedures.
Assisted
For clients who require a helping hand from their door to the vehicle and to the entrance of the facility. This service provides an added layer of safety and support.
Wheelchair
For riders who use a folding wheelchair and can self-transfer. Our drivers provide safe loading/unloading assistance and ensure secure transport.
Medicaid
If you have Medicaid assistance, your trip must be scheduled through your Medicaid brokerage.

Step 2: Decide Your Payment Option
We provide service for Medicaid, Medicare Advantage (through brokerage partners), and Private Pay clients. AGE Medical Transportation LLC operates in Kentucky, and Greater Nashville (TN).
Medicare Advantage
Some Medicare Advantage plans include transportation through their brokerage company.
Please contact your plan to confirm if you have these benefits.
If covered, tell them you would like to use AGE Medical Transportation LLC.
Private Pay
If you are paying with cash, credit, or debit, you may request transportation directly from us — no brokerage approval required.

Step 3: Request Transportation
You can request transportation in three different ways, depending on your preference.
B. Online Booking Portal (Optional)
For clients who have been approved for online booking, you may schedule rides through the portal.
If you would like to set up a portal login, please call us and we will assist you with account activation.
C. Online Request Form
You may also complete our online transportation request form.
Once submitted, a team member will review your request and contact you to confirm scheduling.

Please do not include protected health information (PHI) in the form.
We will verify all medical details by phone if needed.
